Proposed Transportation Basis and Rates-

Each trip will have a travel basis as follows:

Trip A- Point of origin to point of destination

Trip B- Point of destination to point of origin

Trip C- Point of origin to point of destination

Each preauthorized destination or each requested interim destination will be considered as a separate trip. 

Example A- Home to Doctor =       Trip-A

                  Doctor to Home =        Trip-B

Example B- Home to Doctor =       Trip-A

                  Doctor to Pharmacy = Trip-B

                 Pharmacy to Home =   Trip-C

We understand the need to accommodate unplanned interim trips. We simply ask permission to seek authorization from the payment provider prior to fulfilling such requests from passenger or medical providers.

All trips will be measured in mileage using Google Maps for best directions. All mileage will be rounded up or down to the nearest mile.

Example C-3.5 miles = 3 miles.

Example D-3.6 miles = 4 miles

Proposal Statements of Inclusions:

  1. Breakdown of Fees based on services to be provided
  2. All fees listed will represent non-wheel chair assisted (able-bodied) passenger transportation. Passengers capable of transferring from wheel chairs into an able-bodied passenger seat shall be rated the same as a standard passenger.
  3. Ambulance, basic and advanced non-emergency services

  5. Wheelchair Services
  6. Wheelchair accessible vehicles and lift services are planned within the next 120 days. These services will be included as future demand justifies expansion of additional service types.
  7. Cab Services
  8. No services or rates submitted at this time
  9. Interpretation Services
  10. No services or rates submitted at this time
  11. Mileage Cost for any service provisions
  12. Will be submitted with proposals in  Exhibit – A section
  13. Wait time cost and intervals of calculation

8.    Bariatric services cost(if more than one unit it required)

a.  No rates submitted at this time

 9.   Proposed charge penalties for missed appointments or late appointments (When vendor is deemed at fault)

10.  Proposed response time compliance standards on a monthly basis for all individual vendor transports.

a.  Brown Transportation Service will respond to all trip requests within 24 hours. We will adhere to insurance provider (third party) ­­­­­­­­­­­­­­­­­­­­­­­­­­compliance standards and any reasonable or customary requests for transportation; which qualifies as an urgent or high priority. Short notice arrangements are welcomed.

11.  Monthly reports and/or reporting format

a. Monthly reports can be provided as requested; and can be provided in a reasonable Microsoft Office Suite file format ie, spreadsheet, access database, word document, etc.

12.  Pricing payment terms by Contractor

a. Brown Transportation Service (BTS) will invoice insurance provider (third party) at the end of each two week cycle (to be determined) for previously performed transportation services. Insurance provider (third party) agrees to pay BTS for rendered services within 14 days of receipt of invoices. Delay in timely payments will result in late payment fees of 0.5% per day on outstanding and overdue invoices or accounts payable to Brown Transportation Service.
